* By selecting this box and submitting this form, I agree to the terms of this waiver:I hereby agree to participate in the Leo Baeck Pickleball Court Inauguration and Tournament at Leo Baeck Temple and do release Leo Baeck Temple and its representatives from all liability arising out of my participation in this activity. I acknowledge that my participation in the activities entails known and unanticipated risks which could result in physical or emotional injury or damage to me, to my property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity. I expressly agree and promise to accept and assume all risks of such activities. My participation in this activity is purely voluntary, and I elect to participate in spite of the risks. I understand and agree that if I become ill or incapacitated, Leo Baeck Temple and its employees, may take any action they deem necessary for my safety and well-being, including securing medical treatment and I release Leo Baeck Temple and its representatives from all liability arising out of such actions. Specifically, in consideration of the opportunity to attend and participate in the Leo Baeck Pickleball Court Inauguration and Tournament, I agree that I will release, indemnify and hold harmless Leo Baeck Temple(“LBT”) and its affiliates, officers, directors, representatives, agents and employees and all other persons or entities acting in any capacity on LBT’s behalf (the “Released Parties”) from, and agree not to sue the Released Parties for, any claims that I may have arising from, or in connection with, any personal injury, bodily injury, mental anguish, emotional distress, physical, property or other damage that I may suffer from any cause whatsoever related in any way to my participation in this LBT activity, including securing medical treatment for me. Without limiting the generality of the foregoing, I release, indemnify and hold harmless the Released Parties from, and agree not to sue them for any personal injury, bodily injury, mental anguish, emotional distress, physical, property or other damage that I may suffer from the Released Parties’ negligence arising from or related to my participation in the aforementioned activities, including LBT’s securing or authorizing medical treatment for me. I represent that I have adequate insurance to cover any injury or damage I sustain while participating in the activities and understand Leo Baeck Temple relies on that fact. I further represent that I have no medical or physical conditions that could interfere with my safety or health while engaging in the aforementioned activities.
